I'll start.
Have you noticed in movies and TV shows that automobile headlights are always left on?
The car stops, the engine is turned off, the people get out, they close the doors, but the headlights aren't turned off.
Evidently, in Hollywood, batteries last forever. Unless, of course, someone needs to get away quickly because a monster or a bad guy is coming after them. Then, the battery is near dead and the engine won't start...until about the third try when it miraculously fires up...just in the nick of time.
Also, in a similar observation...
Helmets (space suits, deep sea diving, etc.) in Hollywood always have interior lights so you can see the actor. This would be like trying to drive at night with a spot light pointed at your face.

This bugs the living hell out of me. Well no wonder you died, you couldn't see that alien/creature/space debris/etc coming at you. I prefer a bit more realism.

The one that has always bothered me is when the hero or heroes overpower the armed bad guys and then fail to take the weapons. I'd be grabbing every weapon I could find

Just watched shooting sequence with two bad guys & two cops in an elevator, and everyone was shooting.
OC, the bad guys are both shot & both the good guys are unscathed.

Just watched shooting sequence with two bad guys & two cops in an elevator, and everyone was shooting. OC, the bad guys are both shot & both the good guys are unscathed. *sigh* It's getting worse - I was watching "Person Of Interest", and the good guy sneaks into the bad guy's house. As he's walking down a hallway, the bad guy walks by where the good guy just came in at, sees him, draws his pistol and instead of pulling it all the way out and aiming it a guy 8-10 feet away, he fires from the hip, once, missing the good guy completely. Of course, this allows the good guy to whip around and put three shots into Bad Guy's chest. Have you ever noticed the good guys don't get hurt much, but when they do, it's very serious?

Have you noticed that when you're observing the speed limit, the car behind you will always close the gap...? Or when you are trying to maintain a safe distance between you and the car in front of you and jackholes keep merging in front of you, thus destroying your buffer zone.
