This showed up in a YouTube vid by a CGC boardie. The image and info comes from Back Issue #15 from TwoMorrows. I’ll post the answer by Midnight Sunday(9/27)

Surprise! it’s Werewolf By Night #32. Thankfully Len Wein or someone else from Marvel editorial decided to go with Gil Kane for the now iconic cover. Kane also did the 2nd Moon Knight cover on WWBN #33. Ah...cool. The video is from Etanick on the CGC board by the way. He's a good guy.
