CGC has made so many mistakes lately that I can't keep track of them anymore. Here are the most recent examples I've found. If you see more, please add them to this thread.

Any book that has been encapsulated in a universal holder with restoration noted qualifies for this thread (Yes, even GA books). This is an extremely confusing issue for most collectors and buyers. It would be nice to have ALL examples available in one location for easy reference.
